North Bristol NHS Trust has an opportunity for a committed, self-motivated and forward-thinking part-time band 8a pharmacist to help support our Tier 4 (Surgical) and Tier 3 (Medical) weight-loss management services. This post includes weekend working.

You will work as part of the multidisciplinary team to help manage the pharmaceutical needs of patients in both a clinic and ward-based setting. The post-holder would help to support the weight loss management lead pharmacist with the clinics for managing a new generation of weight-loss medication and support Tier 4 patients through their surgical journey from pre-admission to post-discharge.Responsibilities include prescribing both within and outside a clinic environment, leadership and financial and governance reporting. There will also be opportunities to input to staff training and development including pharmacy, medical and nursing staff and involvement with the services provided by the largest acute trust in the South-West of England. 1. 1. Responsible for the provision, management and development of a highly specialist pharmaceutical service to the Anaesthetics, Surgery, Critical Care and Renal (ASCR) Division to achieve a high standard of efficiency andeffectiveness
2. Responsible for the provision, management and development of a highly specialist pharmaceutical service to the Weight Management Team
3. Supporting and deputising for the pharmacist lead for medical weight management
4. Managing the pharmaceutical care of bariatric surgery patients in the pre and peri-operativeperiod,in conjunction with the lead Band 8a Weight Management & Bariatric Surgery Pharmacist
5. Independently assess and prescribe drugs to treat obesity in a clinic setting
6. Plan peri-operative medication management in conjunction with the patient and the wider multidisciplinary team
7. Prepare prescribing information and documents prior to admission and review based on joint decision making with the patient on the day of surgery
8. Clinically screen prescriptions to ensure the safe, efficient, evidence-based and rational use of medicines in line with the BNSSG Formulary and departmental policies so that risks to patients areminimised
9. Contribute to the overall development of the clinical pharmacy service through active participation in relevant meetings. Work closely with all members of the pharmacy department and other healthcareprofessionals.
10. Attend consultant and ward board rounds whenever possible to provide pharmaceutical expertise
11. Demonstrate strong leadership skills across all elements of the role
12. Provide reports on the use and cost of medicines to the surgical directorate as appropriate and analyse these to advise on strategies to ensure cost-effective use ofresources.
13. Write relevant protocols, procedures and guidelines for use within the Weight Management Clinics and Pre- and Post-Bariatric Surgery and surgical specialist environments
14. Provide pharmaceutical expertise to any multidisciplinary working groups considering protocols, policies, guidelines and integrated care pathways in the specialistarea
15. Assimilate and analyse clinical trial information on new pharmaceutical products intended for use within the speciality for submission to the Area Prescribing and Medicines Optimisation Committee (APMOC).
16. Ensure that an accurate medication history is obtained for each patient using the appropriate sources (GP, community pharmacist, patient/carer, clinic notes, nursing home, electronic patient record systems)
17. Assist with prescribing for weight loss clinic patients and inpatients to enable appropriate patient flow
18. Deals with high level pharmaceutical queries and provide advice and information to patients/carers/nursing and medical staff in connection with drugtreatment
19. Provide a comprehensive clinical ward pharmacy service to the speciality and work with stakeholders to optimise resources and improve patient flow
20. Work closely with internal and external stakeholders, where necessary to ensure timely and seamless discharge ofpatients
21. Document all major interventionsmade
22. Investigate and support the recording of adverse drugreactions and support the documentation of patient allergy in appropriate clinical records
23. Audit and risk manage selected areas of practice to promote continuous quality improvement by affecting change in practice. Participate in ASCR Divisional and Weight Management & Bariatric Surgery clinic auditmeetings
24. Maintain and develop medicines management systems in conjunction with patients/carers, ward staff and pharmacy staff. Supervise and contribute to the development of medicines management technicians and/or other dedicated pharmacystaff
25. Contribute to the training of pharmacy undergraduates, medical students, junior doctors, pre- registration pharmacists, pharmacy technicians, allied health professionals and medical and nursing staff as appropriate. Train and mentor clinical pharmacy diploma students on aspects of the specialistarea
26. Act as an appraiser in the Pharmacy appraisals programme
27. Ensure that all relevant procedures are adhered to, reviewed regularly and that all necessary records are completed
28. Undertake to follow all departmental procedures in accordance with current local practices and legislative requirements
29. Provide professional cover in the dispensary and surgical inpatients on a sessional basis
30. Keep up to date with developments in Pharmacy to maintain a high level of professional knowledge and competence. Attend study days and courses as necessary for the development of the service and the individual
31. Maintain a record of CPD
32. Work with the specialty to comply with national and local patient safety drug alerts and initiatives and help with the implementation and recommendation of associated action plans
33. Provide comprehensive pharmaceutical care to bariatric surgery and weight management patients in all clinical areas
34. Undertake any other duties as required by the Director of Pharmacy or their deputy
