Crynodeb
Develop and maintain safe systems of medication supply to clinical areas in association with the Service provider. Monitor processes that medicines are supplied accurately, efficiently and appropriately to meet the demands of individual patients and the Trust. To facilitate the systems for dispensing and supply of medicines, including: inpatient medication, discharge medication, leave medication, monitored dosage systems. Co-ordinate and manage systems of using patients own drugs and one-stop dispensing (as appropriate). To discuss with newly admitted clients their current use of medications, both prescribed and over the counter and document how the client says they take their medication. To review and update individual service user profiles: maintaining up to date and accurate information on medication therapy for individual patients. To liaise with the ward pharmacist and medical team to highlight any differences in the clients prescribed medication when relating this to what/how they state they take their medication and, where appropriate, clarify any discrepancy by liaising with the GP, carer or other agency
