Our client, a respected and well-established domiciliary care service, has an exciting opportunity for an experienced and organised Care Coordinator to join their growing live-in care division. This is a fantastic opportunity to step into a dynamic, people-focused role where every day brings the chance to make a real difference in someone's life.
As the Care Coordinator for the live-in care team, you will be at the heart of service delivery—ensuring that care packages are matched with skilled carers and that everything runs smoothly for both clients and staff. You’ll work closely with the Care Team Supervisor, senior management, clients, and their families to ensure consistency and excellence.
What You'll Bring:
- Previous experience as a Care Coordinator or in a senior care role – essential
- Level 3 Diploma in Health and Social Care (or equivalent)
- A strong understanding of live-in or domiciliary care and the ability to coordinate multiple packages efficiently
- Excellent organisation and time-management skills
- A calm, solutions-focused attitude and strong interpersonal skills
- Full UK driving licence and access to a vehicle – essential

Key responsibilities include:
- Coordinating live-in care packages with care staff across the region
- Building strong relationships with carers and clients to ensure trust and continuity
- Creating and maintaining effective rotas and schedules
- Conducting care plan reviews and risk assessments
- Supporting recruitment and onboarding of live-in carers
- Handling day-to-day queries and problem-solving in a calm, professional manner
- Ensuring all compliance and care records are maintained to high standards
- Working collaboratively with other members of the care and management team
