Crynodeb
To be professionally and legally responsible and accountable for, all aspects of your own work. To ensure a high standard of clinical care for the patients under your management. To hold clinical responsibility for a designated caseload of patients, and to organise this efficiently and effectively regarding clinical priorities and use of time, working without direct supervision. To independently undertake comprehensive and where applicable, standardised assessments of patients looking at all Occupational Performance areas including self-maintenance, productivity and leisure. To formulate soundly reasoned prognoses and recommend best course of intervention, developing patient centred goals to address occupational performance and skill deficits, enabling patients in the areas of self-maintenance, productivity and leisure. To facilitate educational and rehabilitative group sessions for selected patients. To undertake the continuous assessment and evaluation of the treatment programmes and modify the programmes according to the individual patient needs. To be actively involved in patient discharge planning. At all times to be aware of the needs of the patients and their carers and to promote and encourage patient independence and where possible restore functional ability. To assess and manage clinical risk within own patient caseload. To maintain comprehensive and accurate assessment and treatment records in line with legal and departmental and Trust requirements.
