About the team
Housing Solutions – Apsley, Hemel Hempstead, 37 hours
the Housing Solutions team offer specialist assessment across Hertfordshire for major housing adaptations. The team has a blend of community based Occupational Therapists and Community Care Officer Trusted Assessors who complete specialist housing needs assessments to identify major adaptation solutions to enable a person to have better access to the facilities within their homes.
We explore personal strengths, their family and friends’ support networks, and using expert knowledge we connect people with things that will help individuals and families to get on with their lives, achieving prevention, reducing or delaying the need for statutory provision of care services.
Enablement Team – Farnham House, Stevenage – 37 hours
The Enablement Occupational Therapy Team provide support to older people and adults with a disability. We promote personalisation through our assessment process, focusing on the wellbeing of the individual. The Enablement Occupational Therapists deliver enabling programmes. Their input is time-limited, with intervention generally between 4 -6 weeks, this can be extended up to 12weeks where identified goals require this. The care management is completed externally to the enablement team allowing our OTs to focus solely on their OT interventions.
