Crynodeb
Use a patient centred approach to identify individual health goals and develop personalised health plans. Deliver one to one and group coaching sessions to support self management and healthy lifestyle changes. Apply motivational interviewing and other evidence based behaviour change techniques to empower patients. Work as part of the multidisciplinary PCN team, liaising closely with GPs, care coordinators, social prescribers and other ARRS colleagues. Signpost and refer patients to relevant community resources and services. Contribute to multidisciplinary case discussions and population health initiatives. Maintain accurate, timely and confidential patient records on practice/PCN systems. Track outcomes and produce reports that evidence impact on health inequalities and preventive care. Participate in clinical supervision, mandatory training and continual professional development. Adhere to all relevant clinical governance, safeguarding and dataprotection policies
