Responsibilities
Conduct comprehensive assessments of client needs and strengths.
Develop, implement, and monitor individualized care plans in collaboration with clients and their families.
Provide counseling and support to individuals facing a range of issues, including mental health, substance abuse, and social challenges.
Advocate for clients’ rights and access to services and resources within the community.
Work collaboratively with healthcare providers, community organizations, and service agencies to coordinate care and support.
Stay informed about relevant legislation, policies, and community resources that affect client services.
Maintain accurate and up-to-date case records and documentation in accordance with agency policies and legal requirements.
Requirements
Master's degree in Social Work from an accredited institution.
Valid state social work licensure (LCSW or equivalent).
Minimum of three years of experience in adult social work or a related field.
Strong knowledge of adult mental health, substance abuse, and social services.
Excellent communication, interpersonal, and problem-solving skills.
Ability to work effectively as part of a multidisciplinary team.
Strong organizational skills and attention to detail in case management.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:30
Pay Per Hour: £35.00
