Crynodeb
Assessment and management of patients in the community with primarily memory problems who meet the teams inclusion criteria. Joint working, support and advice on the management of appropriate patients to other members of the Memory Team as well as to colleagues in Social Services To participate in care planning meetings To liaise with other parts of the service (especially the wards and IHTT when urgent assessments are needed) . To also liaise with GPs and non NHS organisations when indicated To cover for colleagues during planned absence and short periods of sick leave Supervision of junior medical staff To support with medical leadership and senior clinical support to the multi- disciplinary team. To undertake more complex capacity assessments and apply the Mental Capacity Act and Mental Health Act appropriately To deliver pharmacological treatment in accordance with NICE guidance. To be aware of current research and discuss participation in research programmes with patients
