Key Responsibilities:
Carry out Care Act assessments
Conduct Mental Capacity assessments
Attend Multi-Disciplinary Team (MDT) meetings
Manage safeguarding cases
Work collaboratively within multi-agency teams
Support vulnerable individuals in complex situations
Ensure compliance with relevant legislation and best practices
Provide timely, person-centered care planning and risk assessments
Essential Requirements:
Degree/Masters in Social Work and SWE registration.
2+ years of post-qualification experience.
Strong knowledge of UK social care legislation and practice.
