JOB DESCRIPTION Job Title: Greenwich Health Live Well Health Care Assistant Responsible to: Live Well Lead Pay range: £18,000-£24,000 (pro rata WTE) Location: 25 John Wilson Street, Woolwich, London, SE18 6PZ/Live Well Hubs Hours of work: 25-30 hours per week Key Relationships: Greenwich Health Directors (GHD), Greenwich Health Senior Management Team, Greenwich Clinical Commissioning Group (GCCG), The Royal Borough of Greenwich (RBG), Member Practices, NHS England (NHSE). Greenwich Health Greenwich Health is committed to providing quality to care to our patients whilst building resilience for General Practice and delivering services and programmes of work to deliver care at scale to the Greenwich Population. The post holder will be required to provide support for our Long-Acting Reversible Contraception clinics as well as NHS Health Checks within our Live Well Centres in locations across the Borough of Greenwich. Our Live Well Centres offer Long-Acting Reversible Contraception (LARC) services and this role involves the support of this service by assisting the clinician in clinics and supporting the administration of the service. The role with also involve supporting patients through the service from managing the referral list, triaging, and managing bookings into the clinics. The NHS Health Check aims to help delay or prevent heart disease, stroke, diabetes, kidney disease and dementia. Individuals between the ages of 40 - 74, who have not already been diagnosed with one of these conditions or do not have certain risk factors, will be invited every 5 years to have a check to assess their risk and will be given advice to reduce or manage that risk. The NHS Health Check involves measuring blood pressure and pulse, taking a near-patient blood sample to measure cholesterol, measuring height, weight and BMI and asking questions about lifestyle. NOTE: NHS Health Checks are not currently operational due to Covid-19. This role will support the NHS Health Check Programme once it restarts. Job Summary Process the referrals from practices into the LARC clinics, providing telephone support, triage and provide accurate information to ensure the patient attends the appropriate appointment and is aware of the arrangements. Act as a LARC assistant to our LARC clinical staff, ensuring the patients feel comfortable and the clinician has appropriate, timely support. The postholder would be required to work at weekends as well as during the week to ensure that patient needs are met. Deliver the NHS Health Check sessions across Greenwich within the Live Well Centres. Carry out the NHS Health Check, and support LARC clinics following protocols and clinical guidelines. Create a patient focused environment to help individuals identify how their behaviour might affect their health and wellbeing. Communicate with individuals in line with evidence based practice. Motivate and support individuals to make lifestyle changes via appropriate onward referrals. Prepare, organise and collate own paperwork, equipment and resources ready for delivery. Keep accurate and factual patient records in line with Information Governance, Data Protection Act and Greenwich Health policies. Complete timely and accurate data entry onto the company clinical system. Work alongside the Greenwich Health team to ensure administrative tasks are undertaken according to service protocol. Training Complete all mandatory company training, including annual update / refresher training. Take an active part in developing own knowledge and skills, particularly the key skills and competencies required to carry out the LARC support and to deliver NHS Health Checks. Keep abreast of changes within the health industry via the reading of literature, attending seminars and continuing professional development Health & Safety Monitor and maintain health, safety and security of self and others. Respond to relevant clinical /other issues or incidences to mitigate risk and maintain best practice. Follow information governance and safeguarding procedures. Adhere to all organisational policies and procedures. General Manage and organise own time / activities to ensure timely service delivery to service users. Work collaboratively as part of a team to support service improvement. Seek advice and support as and when appropriate. Be a positive ambassador for Live Well at all times. Contribute towards service meetings, identifying improvements to service delivery. Demonstrate an understanding of equal opportunities and value the importance of diversity.
