Crynodeb
The post holder will deliver high quality, safe nursing care to the practice population. Supported by the Lead Nurse, Registered Nurses, GPs and Practice Management teams they will deliver care within the boundaries of their scope of practice and within the organisations policies and protocols, focusing on supporting patients with health needs, promoting self-care, monitoring long term conditions, disease prevention and screening activities. The successful candidate must be able to work with, and understand the roles of, a variety of different people working in the practice including doctors, nurses, healthcare assistants, social prescribing link workers, health and wellbeing coaches, clinical pharmacists, physiotherapists, paramedics, practice managers and the wider team.
