Key Responsibilities:
Provide supervision and leadership to frontline staff within the hospital discharge team.
Prioritise, screen, and evaluate referrals, case work, and funding applications.
Act as Safeguarding Adults Manager (SAM), taking the lead on safeguarding concerns and investigations.
Ensure effective and timely communication with internal and external stakeholders, including health professionals.
Maintain expert knowledge of relevant Adult Social Care legislation, including the Care Act 2014 (Section 42), Mental Capacity Act 2005, and Equality Act 2010.
Support and contribute to safe, person-centred hospital discharges in a high-pressure environment.
Essential Requirements:
Degree/Masters in Social Work and SWE registration.
Minimum 2 years of post-qualification experience, including recent work in a hospital discharge setting.
Proven ability to supervise, lead, and manage within fast-paced adult social care environments.
Strong knowledge of UK social care legislation and safeguarding responsibilities.
Right to work in the UK.
