Salary is dependent on NHS experience and current banding/pay point, applicants from outside the NHS will receive the start point of the salary scale in line with NHS terms & conditions. Detailed job description and main responsibilities To work within the Nursing Associate parameters of practice and deliver high quality, compassionate care to patients in their own homes/clinic environment. To work with PCN colleagues to deliver care that will be focused on patients who have, Mental Health and Learning disabilities. Or who are at risk of multimorbiditys by ensuring they are monitored and have access to receiving the physical, psychological, and social assessments. To work under the direction/delegation of a nominated clinician, in accordance with an individual care plan, with a focus on promoting health and independence in a personalised care plan using the What matters to me approach. To carry out specific delegated clinical and care tasks and responsibilities to a high standard and competency, under the direction and supervision of a registered nurse or other registered health professional. To proactively monitor, and prioritise patients appointments and visits within a delegated caseload, making autonomous decisions within agreed competencies about patient care and seeking support from colleagues as appropriate. Work in an effective and organised manner demonstrating excellent time management and organisational skills to effectively deliver person-centred care for an allocated group of individuals. To contribute to the development of staff within the service through training and supervision.
