Crynodeb
The role of the Palliative Care Dementia Nurse Specialist (PCDNS) is to enable more people living with dementia to access and benefit from specialist palliative care services. The Palliative Care Dementia Nurse Specialist will: Support people with dementia (and their professional or informal carers, friends and family) to engage in ACP e.g. Identification of what is most important to patients at the end of their lives, a Ceiling of Treatment, and preferred place of care and death. Assess the end of life care needs of patients with advanced dementia and their families including their physical, behavioural, psychological, social and spiritual needs. Work alongside colleagues at Royal Trinity Hospice and other community health and social care agencies to co-ordinate and manage end of life care for patients referred with dementia.
