The Discharge Lounge team
Contributes to the delivery of outstanding patient care by:
Ensuring safe, effective and timely discharge from the trust to another place of care or to their own place of residence.
Communicating openly with patients, their families and other health/social care professionals.
Providing a comfortable and relaxing environment for patients awaiting discharge, where ongoing care needs can be met with support from dedicated members of staff.
Facilitating patient flow through the hospital.
Monitoring discharge progress and escalating any concerns timely to prevent delays/prolonged hospital stays.
Toprovideexcellenceinnursing caretoallpatientsutilising thedischargelounge.
Withsupervisionandsupportfrom theWard Leader,totakechargeoftheday-to-dayoperationofthedischargelounge.
Toprovideprofessional nursing leadership,guidanceandclinical advicetostaffmembers.
Toassistintheeffectiveoperational managementofthewardandflowofpatientswithinthehospital,concentrating ontheprovisionofqualitycareforpatients.
