Duties and Responsibilities Recognise and work within own competence and professional code of conduct as regulated by the appropriate professional body Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures Prioritise, organise and manage own workload in a manner that maintains and promotes quality Deliver care according to local and national guidelines Be familiar with the use of different IT systems and have basic IT skills and EPS skills. Obtain and document consent for any treatment undertaken and keep the patient at the centre of his/her own care, ensuring full participation in decision making Remotely Assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing the services Interpret a range of diagnostic tests from the GP notes to hand and interpret clinical results to aid the consultation e.g. cytology, venepuncture, blood pressure monitoring etc. Work within all relevant policies and procedural guidelines e.g. governance and risk management Safeguarding responsibilities for adults and children and vulnerable groups, reporting concerns appropriately at the correct time on shift, and engaging with services as needed. Clinical Practice Autonomously assess, diagnose, treat and implement care for patients presenting with an undifferentiated diagnosis and those with long term conditions in line with national and local policies and practice/ network guidelines Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan Prescribe medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice Work with patients in order to support concordance with and adherence to prescribed treatments, provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions Promote self-care and health and well-being through the use of health promotion, health education, and therapeutic communication skills Communicates highly sensitive condition related information to patients, relatives and carers When required, ensure patients are referred to other members of the primary health care team e.g. in hours GP/ nurse, d/nurse, or secondary care as necessary Clinically examine and assess patients needs from a psychological and physiological perspective and plan clinical care accordingly Ensure the correct use of clinical templates, read codes and protocols Inform the Clinical Manager / Service Coordinator of any untoward incidents, verbal aggression, criminal damage, safeguarding concerns Our key expectations are: Self-awareness Living authentically Adaptability- Being ready to adjust depending on the situation Openness What you see is what you get Positivity with a real sense of being able to strive for the impossible Generosity of spirit- Everyday should be an opportunity to act with kindness Ability to have fun Taking the role seriously, whilst being yourself
