This is a senior/ band 6 Physiotherapist post. The post holder will work within a dynamic stroke interdisciplinary Early Supported Discharge team in central Norfolk.

The post holder will be responsible for the following:
• Provide high quality specialist rehabilitation to patients following a stroke in their own home/communities, to allow patients to achieve their optimal level of independence and maximum rehabilitation potential.
• Work collaboratively across the Central Norfolk Integrated Stroke Service to ensure a seamless provision of stroke services across the acute hospital, inpatient rehabilitation and early supported discharge teams.
• Work autonomously, carrying out the assessment, implementation, and evaluation of evidence-based specialist treatment programs for patients following a stroke, who may have a range of impairments and complex health needs.
• Supervise and support junior staff and students.
• Contribute to service development and data collection within Central Norfolk Integrated Stroke Services.

Main duties of the job:

Work as an autonomous practitioner to manage a significant specialist clinical caseload using neurological skills

Complete comprehensive patient assessments, and make appropriate clinical decisions following assessment, using evidence-based practice and outcome measures to provide most effective treatment.

Work as part of a multi-disciplinary team, putting patient care and the patient experience first.

Actively participate in service development and contribute to monitoring of standards of practice, and take personal responsibility in your professional development and professional registration requirements

Show passion for providing a high level of care and safeguarding to patients known to our service.

Have the ability to use initiative, communicate with empathy and be an effective team player.

Participate in the development of physiotherapists, support staff within clinical speciality through training, competencies and supervision

Participate in multi-disciplinary team teaching.

There is a requirement to part of the weekend working rota.

It is essential that you hold a full UK driving license. Regular driving across Central Norfolk will be required.

The teamprovides regular peer support and learning opportunities.
