Crynodeb
Delivering patient centred care by: Clinically assess and treat patients using their expert knowledge of medicines for specific disease areas. Completing the Clinical Pharmacist in Primary Care Pathway and become a prescriber to support the Practice Team. Be responsible for the pro-active care management of patients with complex polypharmacy. Provide specialist expertise in the use of medicines whilst helping to address public health and social care needs of patients and tackle health inequalities;. Provide leadership on person-centred medicines optimisation and quality improvement. The role will have a patient facing element either face to face or telephone - including chronic disease management, repeat prescription management and structured medication reviews.
