Responsibilities
Lead and manage complex client cases, ensuring all assessments and interventions are conducted effectively.
Provide mentorship and support to junior practitioners, fostering a culture of continuous learning and improvement.
Develop, implement, and evaluate tailored support plans for clients based on their unique needs and circumstances.
Collaborate with multidisciplinary teams, including healthcare professionals and community organizations, to coordinate comprehensive care.
Conduct regular reviews of client progress, adjusting strategies and plans as necessary to optimize outcomes.
Ensure compliance with all relevant policies, procedures, and regulations, maintaining high standards of practice.
Facilitate training sessions and workshops to enhance team knowledge and skills in client care best practices.
Requirements
Bachelor's degree or equivalent qualification in a relevant field; advanced degree preferred.
Minimum of 5 years of experience in a related practice area, with demonstrated expertise in case management.
Proven track record in leading complex cases and delivering positive results for clients.
Strong communication skills, both verbal and written, with the ability to engage effectively with clients and stakeholders.
Excellent organizational skills, demonstrating the ability to prioritize tasks and manage time efficiently.
Ability to work collaboratively within a multidisciplinary team and build strong professional relationships.
Commitment to ongoing professional development and staying updated with industry best practices.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £30.55
