Job Purpose:
The primary purpose of a Key Worker is to provide comprehensive support and guidance to young people and be a central point of contact for professionals supporting them, with a focus on fostering independence, personal development, and successful transition into adulthood. Key Workers play a crucial role in support planning, report writing and building trusting relationships with young people, facilitating their journey towards autonomy and self-sufficiency.

Responsibilities:
1.Holistic Support: The Key Worker acts as the primary point of contact for young people residing in supported accommodation, offering personalised support tailored to their individual needs, strengths, and aspirations. They work collaboratively with young people to develop and implement support plans that address their physical, emotional, social, and educational needs.
2.Advocacy and Empowerment: Key Workers advocate on behalf of young people to ensure their voices are heard and their rights are upheld. They empower young people to actively participate in decision-making processes regarding their care, housing, education, and future goals, while providing guidance and support to navigate complex systems and access appropriate services.
3.Life Skills Development: Key Workers facilitate the acquisition of essential life skills necessary for independent living, such as budgeting, cooking, household management, and personal hygiene, by creating the young person’s weekly planner based on the actions and objectives identified in the overall support plan. They provide practical assistance and guidance to the young person and colleagues, offering hands-on support and encouragement to build confidence and competence in daily tasks.
4.Educational and Vocational Support: Key Workers support young people in accessing educational opportunities, vocational training, and employment prospects. They provide guidance on educational pathways, assist with college or job applications, and offer practical assistance in developing CVs, attending interviews, and securing work placements or apprenticeships.
5.Emotional and Well-being Support: Key Workers offer emotional support and guidance to young people experiencing challenges such as trauma, mental health issues, or family breakdown. They provide a safe and supportive environment for young people to express their feelings, offer empathetic listening, and facilitate access to appropriate counselling or therapeutic interventions.
6.Transition Planning: Key Workers work collaboratively with young people, management and external agencies to develop transition plans that prepare young people for moving on from supported accommodation to independent living or alternative arrangements. They assist in identifying suitable housing options, accessing community resources, and establishing support networks to ensure a smooth transition and continuity of care.
7.Risk Management: Key Workers assess and manage risks to ensure the safety and well-being of young people under their care. They implement safeguarding procedures, follow risk assessments, and manage challenging behaviours or crisis situations effectively.
8.Documentation and Reporting: Key Workers maintain accurate records of support provided, including progress notes, incident reports, and support plans. They communicate effectively with colleagues and professionals involved in the young person's support to ensure continuity of support and appropriate interventions.
