We are seeking a compassionate and dedicated CSW - Social Worker - Disabilities (Gr 8) to join our dynamic team. Responsibilities
Provide direct support and counseling to individuals with disabilities and their families.
Develop and implement individualized care plans and support strategies.
Advocate for clients' rights and access to appropriate services and resources.
Facilitate communication between clients, families, and service providers.
Conduct assessments to determine the needs and strengths of clients.
Maintain accurate and timely documentation of client interactions and progress.
Participate in interdisciplinary team meetings to coordinate care and improve service delivery.
Stay updated on best practices and policy changes related to disability services.
Requirements
Bachelor's degree in Social Work or related field; Master's degree preferred.
Current state licensure as a Social Worker (CSW).
Experience working with individuals with disabilities in a professional capacity.
Strong communication and interpersonal skills to build rapport with clients and families.
Ability to navigate various health and social service systems effectively.
Knowledge of community resources related to disabilities and advocacy.
Strong organizational skills with the ability to manage multiple cases efficiently.
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £26.25
