In this vital role, you will be the first point of contact for clients seeking social services and support, providing a friendly and welcoming environment. Your primary responsibility will be to assess client needs and coordinate appropriate services, ensuring a smooth transition into our care system. You will work collaboratively with a team of social workers and community resources to deliver effective support, addressing both immediate and long-term needs of individuals from diverse backgrounds. Responsibilities
Conduct initial assessments of clients' needs and situations.
Provide crisis intervention and emotional support to clients in distress.
Develop and implement individualized service plans in collaboration with clients.
Connect clients with community resources and services relevant to their needs.
Maintain accurate and confidential client records and documentation.
Collaborate with multidisciplinary teams to ensure comprehensive care and support.
Advocate for clients' rights and access to necessary services.
Requirements
Master's degree in Social Work or related field.
Current licensure or eligibility for licensure as a social worker.
Strong understanding of social work principles and best practices.
Excellent verbal and written communication skills.
Ability to work effectively with diverse populations and in various settings.
Demonstrated experience in case management and client advocacy.
Ability to work collaboratively in a team-oriented environment.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £28.25
