Crynodeb
To hold professional responsibility for assessing individual care needs, risk assessments, development, implementation and evaluation of care plans for service users with the most complex rehabilitation needs. Being an effective communicator, observer and listener with advanced interpersonal skills, and a refined ability to deal sensitively with this client group Maintaining confidentiality at all times particularly in relation to consent in relation to sharing information, whilst respecting the individual service users rights and the Trusts position on common sense confidentially. Adhere to the Trusts, relevant policies, procedures and guidelines. Ensure good working relationships with service users, carers, family networks, nearest relatives, other professionals and the public. Demonstrating understanding and use of clinical supervision to reflect upon and modify own and others practice. Please see job description for further details
