Job Description:
Consultant in Frailty and Geriatrics
Location: South East
Salary: £105,504 - £139,882 per annum (pro rata where applicable)
Role Overview: Medacs is recruiting for a Consultant in Frailty and Geriatrics. This role involves providing high standards of care in accordance with the National Service Framework for Frailty and Geriatric Medicine and relevant NICE guidance. The successful candidate will be part of the emergency department and frailty assessment unit, implementing an Acute Physician led AMU model supported by specialty in-reach rotas.
Key Responsibilities:
- Provide emergency care for older persons from ED/emergency floor through to discharge.
- Implement frailty identification early in the emergency pathway.
- Cooperate with Clinical Director and Care Group Management on organizational and budget issues.
- Ensure continuity of responsibility for inpatient care.
- Undertake administrative duties associated with patient care.
- Participate in general medical and rehabilitation duties.
- Develop leadership and management skills with education programs and mentorship.
Acute Frailty:
- Provide acute frailty services, admitting frail patients directly to a Frailty ward from A&E.
- Support comprehensive assessment and seamless rehabilitation goals from admission to discharge.
- Lead daily board rounds with a multidisciplinary team.
- Manage frail patients from admission through to discharge, preventing duplication of assessments and reducing delays.
Falls and Orthogeriatric Services:
- Provide a multidisciplinary, multi-agency falls prevention service.
- Operate a Fracture Liaison Service for patients with low impact fractures.
- Lead falls clinics incorporating syncope service, working closely with cardiology.
- Provide Ortho-geriatrics services for hip fracture care.
- Offer comprehensive osteoporosis services with access to all treatments.
Dementia:
- Support proactive advice and management of patients with delirium and dementia.
- Work with Liaison Psychiatry team to deliver National Dementia CQUIN.
Movement Disorders Services:
- Collaborate with Neurology Team and PD specialist nurses to support Clinic based Movement Disorder Services and inpatient liaison.
- Maintain close links with regional Neuroscience center.
Commitments:
- Comply with GMC guidance, clinical risk management processes, and infection control policies.
- Audit clinical outcomes, report clinical incidents, and alert to risks.
- Participate in annual appraisal and foster good team relationships.
- Communicate effectively with all medical and service departments.
- Undertake non-emergency work during premium time as required.
- Respond to patient complaints in line with the complaints policy.
- Support financial sustainability by collaborating on budget issues.
- Provide eight weeks' notice of annual and study leave.
Teaching:
- Participate in teaching and supervising junior staff.
- Recruit junior staff and supervise their education, including educational contracts, regular appraisal, study leave, annual leave, and rotas.
- Enrol on an approved educational supervisor accreditation course if newly appointed.
Research:
- Take on the role of Principal Investigator with full training and support.

Person Specification:
Essential:
- Professional qualifications.
- Full GMC Registration.
- Specialist Registration or will obtain within 6 months of interview date.
Desirable:
- Higher Degree (MD/PhD or equivalent) in Geriatric Medicine.
Skills and Experience:
- Experience in Frailty and Emergency Department Pathways.
Clinical Governance:
- Experience in audit and service improvement.

Other Requirements:
- Ability and willingness to travel effectively between sites.
