We are seeking a dedicated and experienced ASW Social Worker with a minimum of 2 years post-qualifying experience in working with adults to join our dynamic Social Health Care team. This role is essential in providing comprehensive support and care for adults facing a range of challenges, including mental health issues, disabilities, and social barriers. You will work collaboratively with multidisciplinary teams to deliver high-quality services and interventions that promote the well-being, independence, and recovery of individuals. The ideal candidate will have a strong background in adult social work and demonstrate a commitment to empowering individuals through person-centered practices. As part of our team, you will be expected to navigate complex cases, ensuring that clients receive the appropriate resources and assistance tailored to their unique circumstances. Responsibilities
Conduct comprehensive assessments of individuals' needs and circumstances.
Develop and implement personalized care plans in collaboration with clients and their families.
Provide counseling and support to clients facing social, emotional, and mental health challenges.
Facilitate access to resources and services, including housing, financial aid, and community programs.
Monitor and review client progress regularly, making adjustments to care plans as necessary.
Collaborate with health care professionals and community organizations to coordinate services effectively.
Advocate for clients' rights and needs within the healthcare system.
Requirements
A valid Social Work qualification and registration with the relevant governing body.
Minimum of 2 years post-qualifying experience in adult social work.
Strong understanding of mental health issues and the impact on individuals and families.
Excellent communication and interpersonal skills to build relationships with clients and stakeholders.
Ability to work collaboratively in a multidisciplinary team environment.
Demonstrated experience in conducting assessments and developing care plans.
Strong organizational and case management skills to handle multiple cases effectively.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £25.08
