Crynodeb
Conduct face-to-face, telephone consultations with patients presenting with acute, or long-term conditions. Assess, diagnose, and manage patients independently or in collaboration with GPs and clinicians. Provide treatment, referral, and follow-up care in line with national and local guidelines. Contribute to triage and same-day access systems, managing on-the-day demand effectively. Support long-term condition reviews and health promotion activities. Maintain accurate clinical records and documentation using [e.g., EMIS Web]. Work closely with the wider practice team including GPs, nurses, pharmacists, and care coordinators. Participate in clinical audits, significant event reviews, and contribute to practice meetings and quality improvement initiatives.
