Crynodeb
Conducting one-on-one consultations with participants to assess their readiness to exercise and make behaviour changes to improve their health in line with their needs. Providing ongoing support and encouragement to participants throughout their journey with the service. Delivering group face to face exercise classes in the community. Monitoring and evaluating participant progress. Collaborating with other healthcare professionals and community organizations to promote the One You Haringey programme. Maintaining accurate records of client interactions and program outcomes. Liaise with professionals including GPs and primary care staff to promote and generate referrals to the service. Attend outreach events to promote the service within Haringey Managing physical activity content on the OYH App
