Job Summary As a General Practitioner, GP at Pontardawe Primary Care Centre, you will play a key role in delivering high quality, patient focused healthcare services. You will manage acute and chronic conditions, provide preventative care, and work collaboratively with our multidisciplinary team to enhance patient outcomes. Key Responsibilities Clinical Responsibilities: Conduct patient consultations in-person, via telephone, and through online platforms, addressing a wide range of acute and chronic conditions. Provide holistic, evidence-based care tailored to individual patient needs. Diagnose and manage long-term conditions such as diabetes, asthma, hypertension, and mental health conditions. Develop and implement personalised care plans, offering health promotion and preventative advice. Prescribe medication safely and effectively, working closely with our pharmacist-led pharmacy team. Perform and review appropriate investigations, such as blood tests, ECGs, and imaging referrals. Refer patients to secondary care or specialist services where necessary, ensuring continuity of care. Provide womens health, family planning, and minor surgical procedures (if within your scope of practice). Engage with patients in end-of-life care planning and palliative care, working alongside community teams. Teamwork & Collaboration: Work closely with our multidisciplinary team (MDT), including nurses, pharmacists, healthcare assistants, and administrative staff. Attend and contribute to clinical meetings, practice development sessions, and MDT discussions. Participate in clinical audits, quality improvement projects, and patient safety initiatives. Support and mentor junior staff, trainees, and medical students, where applicable. Practice & Professional Development: Maintain accurate and up-to-date medical records in line with GMC and NHS guidelines. Comply with clinical governance, safeguarding, and infection control policies. Engage in continuous professional development (CPD) and maintain GMC revalidation requirements. Take part in Primary Care Network (PCN) and Cluster-level initiatives to enhance service delivery.
