Summary

Key ResponsibilitiesClinical Practice Deliver advanced nursing care as part of a multidisciplinary team, using expert clinical skills and evidence-based guidelines (e.g. NICE). Triage, assess, diagnose, treat, and refer patients appropriately, both in-practice and through home visits. Prescribe and review medications, request investigations (e.g. X-rays, bloods), and develop patient-centered care plans. Coordinate care delivery, ensuring patient and carer involvement. Refer patients to secondary care or other agencies as needed. Care Management Communicate sensitive information with compassion to patients, carers, and families. Provide specialist clinical advice and care plans, including fast-track referrals. Contribute to national targets (e.g. QOF, QI Projects) and monitor care outcomes for continuous improvement. Health Promotion Lead health screening programs and innovate to meet health priorities. Promote health improvement through education, referrals, and evidence-based interventions. Provide assessments and screening services (e.g. BP checks, immunisations, minor procedures). Administration & Professional Duties Maintain accurate, timely clinical records in line with legal and professional standards. Ensure clinical systems are up to date and service claims are accurate. Attend practice meetings and maintain stocked clinical areas. Training & Development Keep clinical skills and knowledge current, ensuring compliance with revalidation. Participate in appraisal, self-assessment, and peer review. Mentor and train staff and students, supporting their development. Maintain a personal development plan. Leadership & Team Management Act as a clinical leader, supporting staff development and promoting best practice. Support the Advanced Practitioner Partner in managing the nursing team: appraisals, staffing, training, and recruitment. Develop and maintain nursing policies, audits, and infection control standards. Produce patient newsletters and nursing communications. Confidentiality & Data Protection Maintain strict confidentiality and comply with Data Protection and NHS Codes of Practice. Share patient information only within authorised frameworks. Health & Safety Comply with health & safety and infection control policies. Identify and manage risks, ensuring safe waste management, sharps safety, and cleanliness. Promote safeguarding practices for children and vulnerable adults. Equality & Diversity Uphold the rights of patients and staff, promoting equality and addressing discrimination. Support individuals in exercising their rights respectfully and inclusively. Quality & Communication Reflect on practice to enhance quality and patient safety.
