Crynodeb
To undertake the following at ward level: drug histories for inpatients, patients own drug checks and medicines reconciliations and POD checks on discharge ensuring accuracy and thoroughness.To coordinate medicines supply activity at ward level, taking into account ward stock, hospital formulary and available medicines. To provide effective communication and coordination with the dispensaries that we work alongside.To undertake patient counselling on selected patients and to work with clinical pharmacist to ensure patients understand their discharge medicines.To undertake, when required the dispensing, checking or handing out of Oncology clinical trials.To help to grow and expand the Oncology Medicines Management Service on the inpatient ward and the day unit.To be part of a sociable and warm team that aim to provide the highest level of care to their patients
