This Role is Subject to: Essential Car Documentation- This post is a designated Essential Car User. Therefore, you must always hold a full valid driving licence and provide your own car for use at work. You will receive the Essential Car User Allowance. DBS (Disclosure and Barring Service) Clearance - This post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check. Information provided by you or the DBS will be dealt with in a confidential manner and in accordance with the DBS Code of Practice which can be viewed here. We have a policy on the Employment of Ex-Offenders and it is available upon request. This is an exciting opportunity to join the Reablement service, these 4 posts for Hospital Social Care Assessors will be based at North Manchester General and Wythenshawe Hospitals. The successful candidates will join a service that prides itself in seeing people achieve their goals and return to independence.The post holders will work within a hospital setting, they will co work with health and social care professionals, to identify and refer people to the reablement service. The post holder will work in a person centred, strength based way to achieve positive outcomes for the individual whilst helping them to maintain their independence, they will provide and promote choice and control to the individual which will help them transition from hospital back to their local community.
The post holder will be required to manage a caseload, undertake social care assessments and reports to ensure effective statutory service support.
The post holder will be required to recognise and identify safeguarding, taking a person centred approach to safeguarding in accordance with the Care Act 2014. You will need to be able to identify and manage risk and where required provide an effective response in line with local policies and procedures.
The post holder will have an holistic approach to work, with a keen focus on reducing inequalities and supporting citizens and their families to develop their resilience and improve their outcomes along the Reablement pathway. The post holder should have a good understanding of how to achieve better outcomes, using a strength based approach when supporting people towards independence.
The post holder will have good knowledge of Reablement and the Hospital Discharge to Assess processes as we are a CQC regulated service, the post holder will have excellent time management and a knowledge of prioritizing a caseload.
The post holder will support the discharge to assess process from Hospital to community identifying cases suitable for a reablement service and initiating the process. The post holder will be a crucial part in maintaining hospital capacity and helping reablement meet key performance indicators.
The post holder should have a knowledge of electronic and manual systems to ensure the maintenance of accurate and appropriate records, this will help achieve efficiencies deliver on local and national performance targets.
The post holder will be required to work closely and communicate with various stakeholders in health, social care and external to identify citizen needs and ensure any onward referrals are made prior to discharge with the reablement service. The post holder will have a good understanding of the care act 2014, safeguarding, mental capacity and risk management to ensure vulnerable adults are supported appropriately.
