Crynodeb
Provide a defined drug and alcohol service to an allocated caseload of service users, having responsibility for the assessment of needs, the planning and implementation of treatment and care to meet those needs as agreed in line with personal recovery goals. Liaise as appropriate with other members of the multidisciplinary team and other agencies involved in the care of individual service users. The role will also require undertaking and delivering defined activities and interventions for a caseload of identified service users, in accordance with the agreed personal recovery plan. This maybe either on a one to one basis, or as part of a group activity.
