• Provide clinical support and direction to community nursing staff within the locality by sharing theirexpertise.  

• Works alongside the Operational Team Lead to ensure staff are competent in their role andnew staffand inducted and supported to reach competencyrequiredfor their role.  

• Implementing evidence-based practice to ensure best patient care.  

• The function of the Clinical Lead is to provide enhanced clinical support andexpertiseto the patients within their locality. 

• Hold a Specialist Community Nursing Qualification.  

• Hold an Independent Prescribing qualification (V300) or being to undertake within 12 months in post.  

• Promoting a service which prevents patients from having inappropriate or unscheduled hospital admissions and helpsfacilitatehospital discharge.  

• Ensure care in their locality is based on sound assessment and care planning processes.    

• I.T. literate to support staff transitioning to mobile working.    

• Positively represent the service both internally and externally. Communicatehighly complexinformation using informal and formal means to ensure people are fully informed of the service.    

• Ensure thatappropriate Statutoryand mandatory training has been undertaken by all team members.  
• Responsible for the retention and recruitment, induction, personal and professional development of staff.  
•   Monitor clinical supervision, induction,preceptorshipand development of staff.
