Summary

Do you have a strong clinical background and demonstrable track record of leading teams and influencing clinicians? This is an exciting opportunity for a dynamic Lead Pharmacist to join our Integrated Pharmacy workforce leading the joint team delivering clinical pharmacy services to the eight GP practices across Wallasey Wellbeing and Moreton & Meols PCNs.

You will lead a team of pharmacists and technicians to ensure patients get the right medicine at the right time and are supported to get the most out of them. As an integrated role this is an exciting opportunity for the successful candidate to shape how PCN clinical pharmacy services can be developed to improve the health of the populations for these practices.

We have embraced collaboration to achieve a system wide overview of the patients’ medicines journey. This helps you identify how processes can be improved when patients move between care settings and become part of the system wide pharmacy family.
As a lead for the PCN Pharmacy team, the role will:
· Operationally and clinically lead a responsive, proactive, patient focused service to both PCNs.
· Lead a team of Clinical Pharmacists and Technicians to deliver an agreed workplan in line with the PCN joint priorities
· Evaluate performance to ensure a consistently high-quality service is delivered, in collaboration with the multidisciplinary team.
· Provide a highly specialist clinical pharmacy service to patients and staff within the PCNs and across the wider system as required.
· Clinically supervise and provide mentorship to the team which will include trainees.
· Contribute to the strategic development of the service delivered to the PCNs, including developing the joint workplan and projects.
· Build strong relationships with community pharmacy and trust pharmacy colleagues.
· Develop and contribute to the integration agenda by supporting improvements in patient care and the development of integrated pathways across the interface.
