Experienced Social Workers should be able to undertake person centred and strengths-based assessments either within the individual’s own home, hospital or Care Home, with the aim of improving client outcomes for the residents of Cheshire East. Additionally, this post will provide a wide range of learning and training opportunities and includes integrated working across the Health and Social Care sector. This post is an excellent opportunity to progress your career and is suitable for any experienced Social Worker.

We are recruiting x2 post for maternity cover, one post is full time at 37 hours per week and the other post is part time at 22.5 hours per week.

The Short-Term Services team works to Home First principles, and in close partnership with colleagues in acute and community health services, social care, and the voluntary sector. The local health and social care economy are working to embed and improve Home First working and with continued service development it is an exciting time to join the team. The team is community based with in-reach into hospital and community bed settings. The team is innovative, friendly, and supportive, and there are good opportunities for training and professional development within the job role

What we are looking for:

Enthusiastic Social Workers who have a good understanding of adult safeguarding, mental capacity and Best Interest decisions making.
The candidate should be self-motivated with excellent communication skills and have the ability to work within the Adult Community Teams and wider multi-disciplinary arena.
Good organisational skills and time management is key to effectively managing caseloads.
The candidates should be able to provide a client centred approach to care and demonstrate sound knowledge through evidence-based assessments and management of support in a variety of situations.
It will be essential for the post holder to work autonomously within the Council’s policies and professional practice, in addition to maintaining accurate and comprehensive client documentation.
The post holder will be based at Delamere House in Crewe working 22.5 hours and in Macclesfield Town Hall 37 hours , covering maternity leave. Hybrid working is available and includes working from home, there may be a necessity to travel to other bases. Cheshire East operates a 'flexible and mobile' working policy.
