To be a member of a multidisciplinary team and share information with teammembers. Liaise with multidisciplinary team and other service providers. Develop rapport with patients/clients using negotiation and motivational skills in encouraging participation and consent for participation in the cardiac rehabilitation programme. Participate in case conferences, meetings and goal planning as directed by senior staff. Communicate either verbally, by letter or in patient notes, with the multidisciplinary team to ensure patients receive the best possible care. Maintain patient confidentiality and demonstrate an unbiased approach to care Communicate with patients, clients and visitors in a polite and respectful manner reflecting their level of understanding and taking into account cultural and language barriers; also promoting their sense of dignity at all times. To communicate and liaise with all members of the multidisciplinary team in a polite and professional manner to contribute to all relevant aspects relating to patient management and care. To empathise, reassure (where appropriate) and to utilise persuasive skills when dealing with patients in highly emotive states. To develop skills to diffuse situations which occasionally arise when dealing with aggressive, demanding or angry patients/carers/relatives. Positively promote the cardiac rehabilitation programme and be involved in all aspects of Health Promotion where appropriate. BACPR Specialist Exercise Instructor Level 4 Cardiac Qualification. Knowledge of the aetiology, pathophysiology, epidemiology, diagnosis and management of cardiac disease and its related comorbidities. Knowledge of exercise pre-screening, assessment, prescription, monitoring, supervision and demonstration for individuals with cardiac disease. Experience in the management of individuals with cardiac disease and related comorbidities. Ensure that patients receive adequate supervision during gym sessions taking into account the patients individualised risk profile. Ability to interpret complex information gained from pre-exercise assessment to determine appropriate exercise prescription for patients and demonstrate the ability to constantly evaluate patients conditions. Ability to adapt and progress exercise programmes, as well as modify overall rehabilitation goals as necessary. To be responsible for own time management, prioritising caseload in conjunction with service requirements. In lieu of the senior exercise staff, maintain the exercise component of the cardiac rehabilitation program. Liaise with other team members for smooth running of service by verbal or non-verbal means of communication email, written instructions, and telephone. Ability to demonstrate fundamental pieces of exercise equipment. Ability to demonstrate the dexterity, co-ordination and sensory skills required to take physiological observations as well as venous blood samples, following appropriate training. Contribute to the Divisions Clinical Governance framework ensuring that all aspects of the service provided is to the highest clinical standard. Organise referral into appropriate exercise programs in primary care. Monitoring, recording and progressing patients exercise programmes in accordance with agreed guidelines, protocols and treatment regime ensuring that individualised exercise programmes are safe, effective and appropriate. Assist with the development and implementation of patient-specific and goal-centred care programmes. Recognise early signs of potential critical illness and take appropriate action Assist with functional exercise capacity testing. To work flexibly by working occasional unsociable hours to accommodate service requirements. To work to the profession specific code of Ethics and code of Professional Conduct. To contribute in the planning and implementation of developments within cardiac rehabilitation. Participate in safety checking of the gym equipment, environment and emergency equipment prior to gym sessions. To ensure all equipment is used correctly and any faults or concerns are reported according to agreed protocols. Use and maintain resources efficiently and effectively and encourage others to do so. Ensure that the sickness and absence policy is adhered to personally. Be involved in internal teaching for non-professionals, patients/carers and new or ess experienced staff. To maintain patient notes and documentation in accordance with procedures. To carry out general administrative and clerical duties. Collection of activity data and interpretation of information to collate activity documentation under the direction of senior staff. Input clinical data to electronic and other information systems To participate in audit and research projects within cardiac rehabilitation. The post holder will work independently whilst maintaining appropriate links with senior staff. To work to the profession specific code of Ethics and code of Professional Conduct. To carry out duties and accept special responsibilities, which are commensurate with this grade. In the absence of the senior exercise staff, oversee the running of the gym ensuring that all appropriate protocols and procedures are followed and care is both safe and of high quality A good level of physical fitness is required in order to demonstrate a wide range of exercise equipment and contribute/participate in exercise sessions. Frequent requirement for moderate physical effort during the day to day running of the gym. Requirement to make repetitive movements when demonstrating exercise equipment. Requirement to kneel, crouch, twist, bend and stretch. Requirement to lift/push/pull and carry supplies and equipment. Requirement to travel between different working sites. The majority of the time will be spent standing up in order to assist patients with gym equipment and supervise the environment. Ability to maintain concentration and supervise a busy gym environment. Deal with difficult and distressed patients in a compassionate manner. Recognise signs of depression and anxiety in patients and appropriate referral practices. Occasional contact with unpleasant smells e.g. body odour/body fluids.
