Summary

Hospital Senior Social Worker Grade 9
Are you passionate about making a positive and lasting difference for the people of Hull? Are you able to contribute to achieving our vision of enabling people to have a life not a service?
You will work as part of an integrated multi-disciplinary team in the Hospital team to provide high quality social work practice; ensuring individuals have a solution focussed plan towards achieving their goals and maintaining their independence. You will work creatively with individuals, local communities and our partners, empowering them to deliver the best possible outcomes for people and to create a positive Adult Social Care culture.
We are looking for individuals who can demonstrate:

a strong value base that informs your work with adults
a relentless commitment to quality - and to be able to talk about what good practice 'looks like'
a capacity for honest reflection and openness to learning;
Ability to establish professional, effective working relationships with a range of partners/colleagues, adults, families and their carers
An individual who can support and help drive the new Discharge to Assess model once it is established.

You must be a qualified social worker and be registered with Social Worker England. This post will provide and support expert and effective social work practice in complex situations, assessing and managing high levels of risk and providing expert opinion in relation to decisions surrounding complex cases. You will undertake a professional leadership role taking responsibility for the learning and development of others, through leadership and management, supervision, mentoring, assessing and teaching.
The Hospital team supports all individuals requiring longer term adult social care input on discharge from hospital; the senior social worker role is the main contact for multidisplinary working and needs to be in the office on a rota basis as part of an IDT supporting referrals and pathway decision making, whilst working with health professionals to assess their immediate needs, identifying the goals they would like to achieve, and the support required to meet these, working with Brokerage to identify suitable providers; ensuring a timely and coordinated discharge from hospital.
This post is a 7 day working post and may be based either in the hospital team or in the community assessing individuals post discharge and you may be required to work between the hours of 8am and 8pm.
We provide a range of benefits to motivate, reward and recognise the valuable work of our employees, which include:

Work/Life Balance - The Council recognises that all employees need to balance home life with meeting work commitments through the provision of appropriate policies and support, for example, family and parental leave, flexible hours and career break schemes.
Learning and Development - The Council has a commitment to lifelong learning. We provide excellent development opportunities for all employees.
Pension - We provide a defined benefit pension scheme whereby the Council contributes towards a pension payable on retirement.
Employee discounts - There are a selection of voluntary benefits available giving a wide variety of staff discounts on goods and services from a range of suppliers via our Hull plus website.
Relocation Allowance available to minimise any financial detriment which may be incurred by newly appointed staff moving from outside the area
Laptops for all social workers, to enable them to work smarter and more flexibly.

For an informal discussion about the role, please contact Kat Fox on 01482 615197.
