Transitions South is a Young Persons (16-21yr old) supported accommodation that operates a ‘Core and Cluster’ model with a total of 27 units. The team offer a Person Centered and Trauma Informed approach to supporting residents with other secondary issues such as substance misuse, offending behaviour and mild learning difficulties.
We are looking for individuals committed to delivering encouragement and support, with the Service User at the forefront of your actions. We want our team to be enabling service users to develop skills and empower them to live independently and avoid relapse. Please note the role does not require the delivery of personal care.
This is an exciting opportunity for an individual who will thrive on and gain satisfaction from supporting others and where you can help shape the development of the service.
You will:
Be able to assess individual service users’ strengths and needs, and work positively with risk
Work with your colleagues to deliver a comprehensive support package, empowering individuals to live independently
Be resilient and manage stressful situations in a calm and positive way
Provide a safe and secure environment for service users
Promote equality and diversity within the service
Be flexible and happy to work on a rolling rota of shift work - 8am - 2pm and 2pm -10pm shifts, inclusive of weekend work.
