Description
The Social Worker - Grade K position offers a unique opportunity for compassionate and driven individuals to make a substantial impact in the lives of clients through advocacy, support, and community engagement. The successful candidate will assess client needs, developing tailored care plans to meet specific circumstances. Working closely with multidisciplinary teams, the Social Worker will facilitate access to essential services such as healthcare, housing, and education, while also providing counseling and support to individuals facing emotional, psychological, or social hardships. The role demands a commitment to social justice and equity, utilizing evidence-based practices to enhance client well-being and strengthen community resources. The Social Worker will also engage in community outreach initiatives to identify gaps in services and advocate for systemic change. With an emphasis on client empowerment, the ideal candidate will navigate complex social systems with empathy and professionalism, ensuring that clients are informed and supported on their journey towards stability and success. Responsibilities
Conduct comprehensive assessments of clients’ needs and strengths.
Develop, implement, and monitor individualized treatment plans.
Provide direct counseling and support to clients and families.
Collaborate with healthcare providers, schools, and community organizations to coordinate services.
Advocate for clients to access necessary resources and services.
Maintain accurate and timely documentation of client interactions and progress notes.
Participate in multidisciplinary team meetings and case reviews to discuss client care strategies.
Requirements
Master's degree in Social Work from an accredited institution.
Current State licensure as a Licensed Clinical Social Worker (LCSW) or equivalent.
Minimum of 2 years of experience in a social work role.
Strong understanding of social service systems and community resources.
Excellent communication and interpersonal skills.
Ability to work collaboratively within a multidisciplinary team.
Commitment to professional ethics and continuing education.
