Crynodeb
To have overall responsibility for managing planned programmes of care for patients in domiciliaryand clinic settings. To assess, plan, implement and evaluate nursing care to patients across thecommunity/primary care interface in the most efficient and clinically effective way, in co-operation withother members of the health and social care teams, and voluntary and statutory agencies.To work with the team leader to allocate appropriate workloads to team members; both registered andnon-registered staff.The post holder will work with a range of professionals in order to ensure care is appropriate anddelivered in a timely manner. The post holder will be able to demonstrate their professional skills andcompetence in leadership, care planning / co-ordination, medicines management, and care andtreatment delivery.To manage workers and resources efficiently and effectively under his/her control, to provide thehighest possible standard of care for patients.To provide advice and support to junior staff in the delivery of high-quality care and actively supportthe team leader in the management of the team.
