Responsibilities
Conduct thorough assessments of clients' needs and circumstances.
Create and implement individualized care plans based on client assessments.
Provide ongoing support and counseling to clients in a variety of settings.
Collaborate with healthcare professionals, families, and community organizations.
Advocate for client needs and rights within various service systems.
Monitor client progress and adjust care plans as necessary.
Maintain detailed records of client interactions and outcomes.
Requirements
Master's degree in Social Work from an accredited program.
Valid social work license or certification in the respective state.
Minimum of three years of experience in social work, preferably with adults.
Strong understanding of social services and community resources.
Excellent communication and interpersonal skills.
Ability to handle sensitive information with confidentiality and discretion.
Proficient in case management software and Microsoft Office Suite.
Hours Per Week : 35.00
Start Time: 08:30
End Time: 16:30
Pay Per Hour: £35.00
