Description
We are seeking an experienced and compassionate Adult Social Worker to join our team. In this role, you will be responsible for providing support and assistance to adults facing a variety of challenges, including mental health issues, substance abuse, homelessness, and domestic violence. The ideal candidate will have a strong understanding of the complexities of adult social work, excellent interpersonal skills, and the ability to work effectively in a fast-paced and demanding environment.
As an Adult Social Worker, you will be responsible for conducting assessments and developing tailored care plans for each individual client. You will coordinate and collaborate with healthcare professionals, community organizations, and government agencies to ensure that clients receive the necessary resources and services. Additionally, you will provide emotional support, counseling, and guidance to clients, empowering them to make positive changes and improve their overall well-being.
Responsibilities
Conduct thorough assessments of clients to identify their needs and challenges
Develop individualized care plans in collaboration with clients and other stakeholders
Provide counseling and guidance to clients, promoting self-advocacy and empowerment
Connect clients with appropriate community resources and services
Monitor and evaluate client progress, adjusting care plans as needed
Collaborate with healthcare professionals, community organizations, and government agencies to ensure comprehensive care for clients
Maintain accurate and up-to-date documentation of client interactions and interventions
Participate in team meetings and case conferences to discuss client progress and coordinate care
Requirements
Bachelor's degree in social work or a related field (Master's degree preferred)
Minimum of [5] years of experience in adult social work
[Valid] state licensure/certification as a social worker [required]
Strong knowledge of adult mental health issues, substance abuse, and domestic violence
Excellent communication skills, both written and verbal
Ability to work collaboratively with interdisciplinary teams
Strong organizational skills and attention to detail
Proficiency in computer applications and electronic record keeping systems
Ability to prioritize and manage multiple tasks effectively
Compassionate, empathetic, and non-judgmental approach to working with clients
Flexibility to work occasional evenings and weekends as needed
Hours Per Week: 35.00
Start Time: 08:00
End Time: 16:00
Pay Per Hour: £12.26
