Crynodeb

The Gender Identity Clinic (GIC) is the largest and oldest gender clinic in the UK, dating back to 1966. We accept referrals from all over the UK for adults with issues related to gender. We accept referrals for patients who are aged 17 years and older onto our waiting list. Patients will not be offered an appointment before their 18th birthday.

We are a multi-disciplinary team, including psychologists, psychiatrists, endocrinologists, speech and language therapists, and nurses. We work together to provide holistic gender care, focusing on the biological/medical, psychological and social aspects of gender.

This role is to ensure that physical health needs of patients on hormone treatment are taken into account. In partnership with other members of the Multi-Disciplinary Team (MDT), the role supports patients’ safe progression through the pathway, co-ordinating liaison with the patient, GPs, practice nurses and other healthcare professionals. This will involve facilitating and following up on appropriate and regular monitoring of hormone therapy with patients and GPs to coordinate the monitoring and management of hormone therapy, and running nurse-led clinics to review progress and health and wellbeing on hormone therapy whilst also providing a check-in service and additional point of contact with the GIC between core appointments.

• To coordinate the appropriate and regular hormone therapy monitoring and physical heath monitoring of clients at the GIC.
• To maintain and populate clinical records and databases of physical health and hormone parameters
• To run nurse-led clinics focused on reviewing client progress on hormone therapy, ensuring it is safe and effective, with support and supervision of the Clinical Nurse Specialists and endocrine doctors
• To interpret routine hormone therapy monitoring results and provide management advice to clients and their GPs in line with the clinic’s Shared Care Protocols
• To liaise with the Clinical Nurse Specialists or Endocrinologist in complex cases, or refer onwards to the endocrine clinic for further assessment
• To ensure effective liaison and communication with both clients and primary and secondary care in providing monitoring and management advice of hormone therapy.
• To follow guidance and use evidence-based approaches in promoting health and well-being and prevent adverse effects on health and well-being
• Appropriate communication skills across the role: with clients and staff.
• To continue to develop own knowledge and skills and act as a resource for the Multi-Disciplinary Team (MDT) in matters relating to hormone therapy and physical health.
