Framework has an exciting opportunity for a part time role in Social Care working during the core hours of Monday to Friday 9am-5pm.
The Support Planner will join a small and friendly team based in Daybrook Nottingham, working out in the community and working from home, while providing a high standard of support to our Service Users.
This is a great chance to work in a Support role where you get to manage your own time. You will have a caseload of around 8 service users that you will be expected to see weekly either in their homes or in the community.
The remainder of your work will be at home with occasional visits to the office. A main feature of this post is the flexibility. You can plan your own hours, and the team are committed to being very flexible with hours.
Elizabeth House Move On is the move-on accommodation from Elizabeth House and provides accommodation and support for homeless people from Broxtowe, Gedling and Rushcliffe. The office is based at Elizabeth House, Daybrook however properties are based in the same 3 boroughs. Some of our properties are based in Eastwood and as such the ideal applicant will need a full driving license and access to a car for work purposes.
We will accept applications from candidates who don't drive but this will likely entail more hours in the office. (Mileage system for car drivers or public transport paid)
We are looking for reliable and self-motivated candidates who can demonstrate an understanding of the issues facing those affected by homelessness.
Duties & Responsibilities
Sign post Tenant's onto appropriate services for substance misuse management & health.
Assist service users with housing applications and move on options.
Assist service users with basic budgeting and life skills in order to support them to move on to live independently.
To liaise appropriately with Key partners, stakeholders and service providers to ensure appropriate support is offered to all Service Users.
To assess, co-ordinate and review the planning and delivery of support to service users.
To be responsible for assessing and evaluating the needs of service users, recording relevant information specific to a person's support needs and coordinating referrals into the service.
To motivate and work creatively with service users to engage with their individual support/resettlement plan.
