A rare and exciting opportunity has arisen to join the CAR-T Clinical Nurse Specialist service at Nottingham University Hospitals.



Chimeric Antigen Receptor T-Cell Therapy (CAR-T) is a revolutionary new cancer treatment that we at NUH are now able to offer to our patients with various forms of relapsed/refractory Lymphoma and Acute Lymphocytic Leukaemia (ALL). This is an exciting time to work for NUH as you will be helping our Lead CNS to shape and develop our ever-growing service.



You will be a Key Worker for patients enrolled onto the CAR-T pathway, covering a large geographical location, including Nottinghamshire, Derbyshire, Lincolnshire and Leicestershire. You will be responsible for the 'work-up' of patients, from referral, to organising apheresis, bridging treatments, and planning admission for cell return. You will support our ward nurses clinically with cell returns, whilst also being a senior port of call for advice, guidance and training.



We envision that approximately 50 patients per year will access our service; this number is only set to rise as further CAR-T treatments are approved. • To work as an advanced practitioner and effectively deliver an expert nursing service within the field of cellular (CAR T) therapy
• To manage a caseload of patients referred for cellular (CAR T) therapy
• To ensure that patients and their families are fully informed and supported by providing clinical and psycho-social support
• To deliver evidence-based clinical nursing practice in line with current research, guidelines and standard operating procedures
• Provide follow-up holistic care for this patient group
• To establish and maintain clear lines of communication within the specialty team, with the multidisciplinary team, providing advice, support and education where appropriate and to ensure cohesive multidisciplinary management of patients
• Have responsibility in ensuring that patients are involved in their care. Providing visible evidence that their feedback is used to maintain standards or implement improvements that have an effect on their care.
• Undertake any other duties that may reasonably be required
• To support the lead CAR-T CNS to provide direct clinical care whilst facilitating modernisation of patient services and deputising in their absence



The post holder is required to hold a valid first level registration with the Nursing and Midwifery Council.

The post holder will be based at The Department of Clinical Haematology, City Campus.

1.0 Clinical

1.1 To work under the lead Clinical Nurse Specialist to deliver a specialist nursing service for CAR-T.

1.2 To manage a defined patient caseload, ensuring that all patients have a holistic assessment of needs and a care plan to meet these, involving other healthcare professionals as appropriate.

1.3 To provide direct clinical care, including the infusion of CAR-T cells on the Bone Marrow Transplant ward.

1.4 To coordinate the patients ‘work-up’ prior to CAR-T therapy, including arranging various investigations prior to treatment and timely scheduling of apheresis with bridging treatments and planned admissions.

1.5 To be a visible member of the Multidisciplinary team (MDT), Quality Management team (QM) and CAR-T operational planning meeting, along side other relevant meetings.

1.6 To ensure continuity of a high standard of evidence based nursing care, assessing health, health related and nursing needs of patients, their families and other carers by identifying and initiating appropriate steps for effective care.

This can include:

· Managing patient caseloads

· Ordering and coordinating diagnostic tests

· Making and receiving referrals

· Supporting with nurse led clinics where indicated

· Key role in MDMs

1.7 To be actively involve in in-patient care, liaising with ward staff to ensure high quality nursing care, including assisting with discharge planning to ensure a safe and appropriate follow up.

1.8 To attend outpatients, ensuring nurse consultation is available to all newly referred patients. Also supporting in consultant-led clinic appointments and the running of nurse-led clinics.

1.9 To follow-up patients as appropriate and ensure all relevant information is available to enable patients to make an inform choice about their treatment.

1.10 To plan appropriate investigations and coordinate these as to optimise the patient experience.

1.11 To participate in and support the lead CNS in the process of reviewing relevant operational policies annually in accordance with national standards.

1.12 Utilising a specific body of knowledge, works within a clinical team to deliver a service based on principles that are research based and which enhance the quality of patient care.

1.13 Assess and interpret common conditions related to the patient / client group and within scope of practice.

1.14 Uses complex clinical decision making, managing some risk but deferring to others for overall plan.

1.15 Be a visible point of contact for patients, visitors, relatives, and staff, acting as the patients advocate and an expert resource for patient, staff and students.

1.16 Ensure that patients’ records and documentation are maintained and current, with changes implemented and in accordance with NUH ‘Clinical record keeping policy’.

1.17 Organise meetings to discuss patient management as required.

1.18 Provide specialist support to clinical team in dealing with the complex needs of different client groups.

1.19 Liaise closely with other health and social care professionals to ensure a coordinated approach and high quality care is provided for patients.

1.20 Discuss alternative treatment options and facilitate clinical management decisions.

1.21 Obtain informed consent for procedures as required, in accordance with the Trust consent policy.

1.22 Participate in research studies in conjunction with the specialty team.

1.23 Promote and support personalised care in line with the NHS long term plan including the components of the comprehensive model for personalised care.

1.24 Provide clinical support to wards/departments in times of increased clinical pressure or staffing shortages as requested, ensuring that patient safety is the priority at all times.

1.25 To actively involve patients in providing feedback of their experience of the current service and suggestions for improvements.



2.0 Education, Learning & Development



2.1 Undertakes, and can assess those undertaking, any ‘Working in New Ways Packages’, which have been identified as integral to the role requirements and specialty area.

2.2 Provide specialist clinical knowledge and advice, taking an active role in self-development, reviewing own performance, identifying own needs and taking active measures to meet those needs.

2.3 Carry out formal and informal teaching within the hospital to a range of health care professionals and contribute to educational and training programmes as requested / appropriate.

2.4 Create a positive learning environment within clinical settings to encourage and facilitate individual and shared learning and professional development.

2.5 Participate, in the education of patients and the general public.

2.6 Maintain an awareness and understanding of relevant, up to date research studies within the clinical field.



3.0 Leadership and Management



3.1 Support the senior team as appropriate.

3.2 Deputise in the absence of the lead CAR-T CNS.

3.3 Respond and contribute to clinical governance initiatives eg complaints, datix.

3.4 Initiate and implement change effectively in response to clinical or organisational demands. React positively to, and facilitate the process of effective change through a culture of Shared Governance and frontline leadership.

3.5 Be a representative at a local level and participate as a member of the senior nursing team/ Speciality, making positive contributions to the strategic planning of the service.

3.6 Ensure clinical supervision within the clinical area or department is actively supported and staff encouraged to avail themselves of it.

3.7 Contribute to the development of team objectives.

3.8 Participate in interviewing and selection of new staff.

3.9 Be aware of the impact of service changes across the organisation.

3.10 Participate in managerial duties at department level as required in the absence of line manager.

3.11 Assist with the identification and facilitation of clinical audits to promote clinical effectiveness as part of the Directorate’s Clinical Governance Framework.



4.0 Service Development and Use of Resources



4.1 Adopt innovative ways of working within the speciality.

4.2 Share and promote good practice through involvement with local groups, including presentations, publication and dissemination of role evaluation.

4.3 In collaboration with the Service Lead and colleagues, ensure that the patient has a satisfactory experience by assessing and evaluating patient processes and redesigning the pathway of care when appropriate.

4.4 Contribute to the monitoring and control of expenditure and the use of resources.

4.5 Support in objective setting and business planning for the service.

4.6 Implement change in response to clinical, organisational and national demands; in agreement with appropriate others in the unit.

4.7 Respond to change initiatives in a positive manner.

4.8 Participate in the development of evidence – based patient care guidelines, protocols, policies and procedures where appropriate and ensure they are implemented.

4.9 Ensure that the clinical team or department is kept informed of the policies, direction and strategy of the directorate and division.

4.10 Appraise and evaluate research findings and support the application of these to clinical practice.

4.11 Take a facilitative role in developing practice through specific research and audit projects.

4.12 Collect and analyse information related to patient and carer satisfaction and evaluation of the service.

4.13 Develop a culture that ensures contribution to research and the use of evidence to support innovation and practice at ward/department level.

4.14 Contribute to the production of an Annual Report each year, ensuring it is submitted to Matrons and Divisional Team.



5.0 Communication and Professional Behaviour



5.1 Be a role model to ensure that the dignity, rights and confidentiality of the patient and individual team members are protected and maintained in accordance with Trust and national policy on Information Governance.

5.2 Convey and present complex and sensitive information, written or verbal, formally or informally, as appropriate. Report appropriate clinical information to senior staff. Ensure that written documentation is kept in respect of all investigations and incidents.

5.3 Establish professional relationships and maintain effective communication.

5.4 Link with professional networks across the Trust and wider health community, in order to develop and exchange ideas for improving clinical care.

5.5 Provide ongoing liaison and support for patients /clients within care pathways.
