To be the Specialist Lead Occupational Therapist for Specialist palliative care, based at Pricilla Bacon Hospice, delivering highly specialised Specialist Palliative Care assessments, discharge and advance care planning support to patients attending both inpatient and day service and service development to support patient care across Norfolk.

To work as part of a multi-disciplinary team to assess patients attending PBH and supporting both patient and family. To act as a point of knowledge & support for complex issues related to palliative and end of life care for OT colleagues in the wider community.

To lead, in collaboration with, Nursing and Psychology Discipline Leads and Clinical Lead colleagues, the development of clinical standards, policies and service improvements for this client group, across NCHC.

To provide specialist clinical supervision to the Occupational Therapists and assistants, and general advice and support to the wider multi-disciplinary team within PBH

To review and develop the training and workshop sessions that are offered to colleagues and other professional groups.

2. To work closely with patients/families and other members of the MDT for the best possible outcome for the patient

To complete independent assessments, utilising highly specialised observation and assessment skills for the patients group in line with National Standards To ensure where possible patients can be discharged to their preferred place of care/death in a safe and timely way. Be proactive in supporting patients to complete Advance care Plans

· 4.To support colleagues and provide supervision & education as required: to support HR issues within the OT team within PBH

5. To design, develop and deliver therapeutic sessions for out patient groups and support other staff to deliver them as needed

6. To liaise with multi professional agencies and act as a specialist information, advisor, and training resource to colleagues in other services.

7. To promote patient and public involvement by involving patients, carer/ relatives in planning and delivery of care/ development of services

8.Provide specialist knowledge, support and training to colleagues related to palliative and end of life conditions.

TClinical

· To complete independent assessments, utilising highly specialised observation and assessment skills for those patients referred with life limiting conditions in line with National Standards( eg The 6 Ambitions for Palliative and End of Life Care) and NICE guidelines.



· To be professionally and legally accountable for all aspects of own clinical work and the management of cases on own caseload.

To be responsible for most complex cases and the delegating of work to SPC multi-disciplinary colleagues.



· Participate in administration and interpretation of specific assessment tools within SPC assessment and communicate the results to the SPC MDTs as part of the clinical formulation of individualised plans for the patient and family

· To write detailed and comprehensive clinical reports to support all decision making.

· To act as an expert Occupational Therapy resource within the service in respect of this client group, working autonomously to support patients, families, and other health care professionals.



· Provide specialist knowledge, support and training to colleagues related to palliative and end of life conditions. Support the team with complex clinical decision making around assessment outcomes.

· Be proactive in supporting patients to consider their future wishes and complete Advance Care Plans

· To make clinical decisions around diagnostic and observed outcomes using specialist knowledge and assessments and communicate the results to families and carers in a sensitive way.

· Offer patients and their families information on a range of therapeutic approaches/strategies as part of the assessment process. Including awareness of signposting to relevant organisations.

· To contribute to development of leaflets/ information regarding palliative conditions and services offered by the organisation and associated needs.

· To design, deliver and review specialist outpatient-based group therapy sessions. To lead on these sessions and ensure continuity planning by educating colleagues to support these groups.



· To liaise with multi professional agencies and act as a specialist information, advisor, and training resource to colleagues in other services. To provide specialist knowledge, support and training to clinical colleagues, patients, families, carers, and other relevant agencies.



· To attend various multi-agency meetings as required to advise, write reports and plan care, acting as the patient advocate when appropriate and to work in accordance with the Trusts and PBH’s policies. Be aware of safeguarding issues and ensure Trist policies are followed accordingly.

· To promote patient and public involvement by involving patients, parents/ relatives in planning and delivery of care/ development of services

To contribute actively and regularly to the clinical training of undergraduate Occupational Therapists.



Leadership:



· To provide leadership and clinical supervision to occupational therapy colleagues within PBH and from the wider community if related to palliative care



· To work closely with clinical and team leads in service development ideas to develop the SPC services (both inpatient and outpatient) and quality of care delivery.



· To liaise closely with clinical and team leads to plan occupational therapy input to the SPC inpatient and day services pathways.
