About the role We are seeking to recruit a school to college Transition Coordinator to join our central learning support team, working with young people with an Education, Health and Care Plan (EHCP) who apply or are interested in studying at York College. You will establish working relationships with our feeder school SENDCos and Local Authority caseworkers, to effectively transition students with an EHCP into college.
About the Person You will have experience of working with neurodivergent young people with an EHCP and understand how to review the EHCP to assess a young person’s support requirements. You will have a good working knowledge of the SEND Code of Practice and our duty to admit students with an EHCP. You will be highly organised, with the ability to manage and prioritise a caseload. You will demonstrate excellent interpersonal and communication skills and be able to represent the college in meetings with professionals, parents and young people.
