Crynodeb
To work within the Wellbeing & Outreach team and alongside other internal teams to increase the reach of our palliative care services across all the demographics of the local population, helping ensure the service meets those peoples ongoing needs. To understand the voice of the community and those with lived experience and bring the learning back to the organisation. Main duties and responsibilities Create and maintain strong relationships with external organisations, community groups and community members to raise awareness of existing services. Build connections and relationships with community groups, voluntary groups, faith organisations within the local area. Continue to improve St Leonards understanding of the community we serve and bring back the voice of the community and those with lived experience to the organisation. Act as a service user advocate and facilitator to resolve issues that may be perceived as barriers to care by providing information and support to the local community. Receive highly complex and demanding information and deal with distressing situations in a sympathetic, calm and supportive manner. Escalating issues to the correct services or other health care professionals where appropriate. Support referrals to the Hospices clinical services. Support Wellbeing & Outreach groups such as: Wellbeing Hubs, bereavement groups, disease specific socials and one to one support to service users on a virtual caseload. Be responsible for working alongside and/or supporting volunteers within the team. Manage day to day administrative functions of the service e.g. arranging of meeting/diaries and incoming post. Use a degree of initiative, prioritising workload and reviewing work practices in accordance with service development.
