Summary
The successful applicant will join Garden House Hospice Care, (GHHC) at a very exciting time. Since January 2024, GHHC started an innovative service to support patients with moderate to severe frailty and two or more long term conditions, who would benefit from our services. This service has been featured on the BBC news and radio and GHHC has been invited to give a presentation at the Hospice UK Annual Conference. To support this service for life limited frail patients, previously underserved by palliative care, GHHC increased inpatient and community beds and staffing, including appointing an in-reach nurse. The frailty in-reach nurses role is to identify suitable patients at the Lister Hospital, working collaboratively with the Lister Hospital teams.
