Summary
The integrated care practitioner role is wellestablishedwithin Bracknell. If you are energetic, a team player with a good skillset for autonomous working and decision making; you will be the right person to come and work as part of theIntegrated Care Team.
The role is truly situated in between the services to prevent people falling between the gaps; you will be able to guide the person on correct pathway so there are “no wrong doors”.You are working closely with community nursing colleagues as well as local authority colleagues from Bracknell Forest, discussing new referrals and completing home visits.
This is a hybrid role, whilstbeing able to work from home,you’llalso have opportunity to work at Bracknell CMHT OA,Church HillHouseand Bracknell Forest Council, Time Square.
You will be a well-respected member of Bracknell CMHT forOlderAdultsand the Integrated Care Team,this is a matrixmanagement model -youroperational supervisionwill be provided bythe service manager of theIntegratedService, your clinical supervision will be provided by the service manager fromBracknellCMHTOA.
Theintegrated careextends to working closely with partners, includingGP’s and community nursing teamswhen discussing more complexpatientsin cluster meetings.
The team has access to local authority training as well as Berkshire Healthcare Foundation Trust training. Bothprovidegood opportunities for external training.
• TorepresentBHFT mental health services at locality LAP/Cluster/Care Homemeetings. To screen referrals,identifyany further information which needs more exploration to be able toidentifya correct pathway.
• To provide clinical leadership, and offer mental health expertise & guidance to professional colleagues andassistin formulation to support integrated decision making and careprovision
• To meet the multiple and often complex needs of frail patients with long term conditions who are discussed,through on-going joint assessment of health and wellbeing, including risk assessment, developing,implementingand evaluating intervention plans.
• To support the multidisciplinary team toidentifywhich patients discussed couldbenefitfrom mental health intervention.
• To work as an autonomous practitioner, taking responsibility for mental health assessmentsestablishinga formulation and implementing the clinical managementrequired.
• For those patients not requiring secondary mental health input, to support navigation of the health and social care system by recommending the mostappropriate levelof psychological support from the range of provisions available.
• To hold a small caseload requiring more complex mental health support.

The “must haves” for you to be considered for this role:
1. Degree in Mental Health Nursing or equivalent professional qualification or Dual QualificationegRGN
2. Experience of clinically supervising junior and unqualified staff and mentoring students.
3. Experience of working effectively in a multi-disciplinary team and multi-agency environments.
4. Exceptional interpersonal, influencing, communication,observationand reporting skills. Ability to engage and communicate with a wide range of professionals,staffand other servicestakeholders.
